The route begins as an undivided suburban arterial, starting at U.S. Route 40
in downtown Baltimore. At this point, it is known as Pennsylvania Avenue, though this section is not officially a part of MD 140. Pennsylvania Avenue is known to many as historic "African-American Main Street" of Baltimore .
The MD 140 designation begins after intersecting US 1
(North Avenue) and nearby Fulton Avenue; MD 140 changes its name to Reistertown Road. After intersecting the eastern terminus of MD 26
, it runs northwest out of Baltimore when passing Fallstaff Road through Pikesville and Owings Mills into Reisterstown. After it crosses the Baltimore Beltway, MD 140 is paralleled by Interstate 795
, which is a freeway bypass of the road. It junctions MD 30
in historic Reisterstown and then turns west, meeting the northern terminus of I-795 and upgrading into a four-lane undivided highway. After intersecting MD 91
in Finksburg
it upgrades into the Westminster Bypass, a four-lane divided expressway replacing the original two-lane rural road, which still largely exists as loops and frontage roads of the Bypass. The expressway runs northwest through Westminster, where MD 97
joins it. The two routes run together through Westminster as a heavily congested six-lane arterial; the route was originally constructed as a bypass of the heavily congested Main Street (now unsigned MD 32
for most of its length); the lack of access controls has resulted in overdevelopment.
The two routes split northwest of the central business district, at Pennsylvania Avenue, where MD 27 travels north on said road; MD 140 continues straight and downgrades to a two-lane road with shoulders for the remainder of the route, carrying the name Taneytown Pike. It intersects the northern terminus of both MD 31
and MD 32
. It travels northwest to Taneytown
and passes through it on Baltimore Street. Beyond expanses of farmland, it interchanges with US 15
and touches down in Emmitsburg
on Main Street. Shortly after this, the road, now known as Waynesboro Pike, exits Maryland, becoming Pennsylvania Route 16
.
Until 1979, MD Route 140 was U.S. Route 140
. Before U.S. Route 140 was deleted, it was where MD 140 is today. At the divergence of MD 140 and MD 97 northwest of Westminster, the two routes were swapped after the deletion of the U.S. route; MD 97 now follows US 140's old route to Pennsylvania, while MD 140 follows MD 97's original route to Taneytown
and US 15
. As a result, MD 140 follows the bypass of the original MD 32
between Westminster and Taneytown that was first designated as part of MD 97.

Along Pennsylvania Avenue in Baltimore, service is provided by Route 7. This line operates between Mondawmin and Canton. It also passes the Penn-North and Upton Metro Subway Stations, located on Pennsylvania Avenue.
Between Mondawmin and Old Court Road, service is provided by Route 53
. This line operates every 10–33 minutes, and it overlaps with Route 59
between Patterson Avenue and Old Court.
From Patterson Avenue to the end of Reisterstown Road in Glyndon, Maryland
, service is provided by the Route M-9, which has a deviation to the Owings Mills Metro Subway Station and Town Center along Painters Mill Road. Route M-9 ultimately loops along Maryland Route 30
, also known as Hanover Pike. Service on this line is provided every 20–45 minutes at most times.

Besides the famous 7 Mile Lane, which is 7 miles (11.3 km) from the center of the city, there were once 5 miles (8 km), 10 mile, and 15 miles (24.1 km) houses that dotted the road respectively.
