Plot

The films begins with reality show contestant Kimberly Caldwell

 (playing herself) driving through the West Virginia back country searching for the location of her next project. She turns onto a sideroad signposted to an old paper mill and accidentally hits a deformed teenager ("Brother"). She stops to check whether he's alright, but when she approaches him, he kisses her and rips off part of her face. She stumbles backwards and bumps into "Three Finger" (who apparently survived from the first film), who chops her in half down the middle with an axe. The two drag her body halves off.

The introduction of the show Apocalypse: Ultimate Survivalist is then shown. Six contestants, including Kimberly, must survive in a forest in a scenario intended to represent the aftermath of a nuclear apocalypse. The reality show is hosted by retired U.S. Marine Colonel Dale Murphy (Henry Rollins

), who makes them give up all outside communication before starting. The other contestants are Nina Papas (Erica Leerhsen

) steps into her place. The contestants split into three pairs and move into the forest to begin the contest; the last person "alive" is to win a prize of $100,000.

In the forest, Three Finger attacks and kills Neil, one of the television crew, by slitting his throat and then scalping him. Afterward, another cannibal ("Pa") punctures Dale's ATV tires with arrows, preventing him from escaping. Dale then throws a knife that hits Three Finger in the shoulder. He pulls it out and licks the blood from it. Dale turns around only to be knocked out by Pa.

), Mara's boyfriend and the show's director and cameraman, tries to create "sparks" between Elena and Jake. Elena, who has been briefed beforehand and is hoping to use the show as a springboard to a career, bares her breasts and tries to seduce Jake. Jake, however, refuses to engage in any sexual activity and leaves to look for food in the woods. Elena then fellates

 M. Mara and Nina, who have bonded, watch in disgust from hiding. Mara is devastated and after they find a log cabin in the woods she enters to try to find a telephone to get a lift out of the woods. There they witness a female cannibal ("Ma) giving birth to a terribly deformed baby, and are in turn spotted by her teenage daughter ("Sister"). The girls get out of the cabin through the toilet pit as Sister kicks down the door. The two run away into the woods with the cannibals chasing them. During the pursuit, Mara is hit in the head and killed with an axe thrown by Pa as Nina hides. Pa looks for her but fails to find her. The other cannibals drive up with Mara's body tied to the hood of their truck and Pa gets in and drives off. Nina sets off to find the others.

M returns to the RV and Elena stays there to tan in her underwear. Brother, watching her in the bushes, starts masturbating but is interrupted by Sister, who is furious and jealous of Elena. Sister knocks him over and he sets off an alarm which Elena hears. Starting to get dressed, Elena sees Sister coming out of the bushes towards her, tries to run, but trips and falls into the lake. She is then attacked and killed with a machete by Sister.

While M is in the RV, the cannibals steal it, impaling M with an arrow. Three Finger tortures Dale, who is suspended upside-down, and as he prepares to kill him, Dale hits him between the legs, uses the moment to his advantage, manages to escape, and he and Three Finger battle. After a short fight, Dale shoots Three Finger with a shotgun.

Jake finds Amber and Jonesy at the show's basecamp. As they eat meat that they have discovered cooking on a spit, a crazed Nina races up and tells her story. Jake then realizes it was Kimberly's leg that they had been eating after spotting her signature snake tattoo. As they run, Dale finds an old man (from the previous film) in his cabin. The old man tells him how the cannibals' mutations were caused by effluent dumped in the river by the now-closed paper mill and then reveals that he is their father and attacks Dale. After a brief skirmish, Dale kills the old man by blowing him up with a stick of dynamite.

After Amber, Jonesy, Jake and Nina notice that the RV is gone, they run towards the woods and hear moaning sounds. They see Brother apparently raping Elena. Jake tries to intervene, and it is revealed that the woman is actually Sister wearing Elena's scalp. Nina runs, horrified, and falls into a pit. Amber, Jonesy and Jake fight the cannibals and Brother falls and hits his head on a rock. Jake searches for Nina while Amber and Jonesy go for help. Jake rescues Nina from the pit and they flee to the river, where Sister finds them and shoots at them as they jump in.

After a chase, Amber and Jonesy get caught in a trap and are soon killed by Brother with a single headshot arrow. Meanwhile, Nina and Jake make it to the mill. They walk through the first gate and Nina sees a tank with a toxic symbol labeled "may cause birth defects." Jake and Nina then enter the mill, finding that it is full of vehicles stolen from victims over the years. They find the RV and hear M screaming from inside. Jake enters the RV and realizes that M's screams are actually coming from a live feed monitor inside. He watches as M is decapitated. As Nina waits outside the van, she hears a giggle and Sister appears. Nina is then dragged away. Jake runs outside and is overpowered by Pa.

The abandoned mill is revealed to be the mutant's home. Dale sneaks into the compound. Jake is strung up and Nina is strapped to a chair with barbed wire at the table. The mutants eat dinner, also forcefeeding Nina.

Dale makes a noise and alerts the family, who search for him. He manages to kill Brother and Sister with dynamite attached to an arrow. He frees Nina and, while helping Jake, is shot in the back with an arrow by Pa and is then killed by a razor-wire chain thrown around his throat by Ma, who is incensed by the deaths of her children. Nina successfully escapes, but Jake wanders into a room fitted an enormous tree de-barker, which is used by the cannibals to dispose of the waste from their victims. He soon gets into a fight with Pa. Nina returns to the mill and she and Jake manage to kill Ma and Pa by pushing them into the de-barker. They then find Kimberly's abandoned car and drive away to safety.

Meanwhile, as the film ends, Three Finger is shown feeding the newborn mutant baby Mara's finger and a bottle filled with a greenish chemical waste. His laugh is heard over the end credits.

Uncle Creepy of DreadCentral gave the film 8 out of 10 stars, stating that the film "is a hot ticket for some gore-soaked backwoods mayhem that gets even better with repeated viewings and lots of booze".
Bloody-Disgusting stated that "what could have been a cheap and lazy cash-in turned out to be one of the year’s better genre offerings".
