The region has separate men's league structures for First, Second and Third XIs, and an open Central League for Fourth XIs and below. The champions of the top division are promoted to the Conference level of the England Hockey League (EHL).

For the 2009-10 season, the six leading clubs in the Second XI Midlands Premier Division have formed a new Midlands Challenge league, separate from the MRHA's league structure, to act as a development league for national league players.
