The event was formerly known as the Premio Ellington, and for a period it held Group 2 status. It was renamed in memory of Carlo d'Alessio (1915–1994), a successful racehorse owner, in 2000. D'Alessio's horses included Bolkonski, Le Moss, Tolomeo and Wollow.

The Premio Carlo d'Alessio was extended from 2,400 to 2,800 metres in 2001, and it reverted to the former length in 2005. It was downgraded to Group 3 level in 2006.
