When Rex goes on a mission to defeat the EVO, Multi-Headed EVO, he discovers a piece of a blueprint which later is stolen by Breach. This causes Rex to lose some of his new builds. He reports this happening to Providence and goes to the Jungle. A piece of the blueprint is located in Dr. Rylander's lab, where Rex copies Rylander's data. He is then quickly attacked by the "Rabbit EVO", who he later defeats. Breach steals the blueprint again, as Rex returns to Providence. Afterwards, Rex goes to Hong Kong
with Bobo Haha to stop Quarry (Rex's original boss) from stealing items, including a piece of the blueprint. To Rex's interest he encounters his romantic love, Circe. He finds out that she is working for Van Kleiss, again, as he helped her escape from him.
Rex stops Quarry but he falls into a river, unable to get the blueprint. Rex leaves Hong Kong right when Quarry gets out of the river and gives Circe the blueprint. It is later revealed that Circe is helping Van Kleiss to only become a normal girl again, when he builds a "Omega One" Nanite. Rex's older brother, Caesar, tells Rex that Van Kleiss can never make the Omega-One Nanite, because he has one quarter of the blueprint memorized in his mind. That quarter was never copied down on paper. Suddenly Doctor Holiday is attacked, to stray Rex away from his brother, so Van Kleiss can kidnap him. Rex later sets out to find the rest of the blueprints, going to Aquania (whose Heart is transformed into an E.V.O.) where he is suddenly kidnapped by Breach.
Rex is transported to Breach's private pocket dimension, Greenville, Ohio, when he escapes (with the blueprints) he goes to Abysus to confront Van Kleiss. Unfortunately, Circe steals Rex's blueprints when kissing him. Van Kleiss creates his Omega-One Nanite, resulting with him becoming a giant E.V.O. However, he loses control of the almighty, divine Omega-One Nanite, when Rex explains that he tampered with the blueprint. Rex defeats Van Kleiss, with the help of Circe, and returns to Providence where he and Bobo decide to go partying.
A total of seven different Nanite builds for Rex such as Rex's Slam Cannon, Smackhands, Big Fat Sword, Punk Busters,Blast Caster , Battle Axes (known as "Bad Axes" in game) and Funchucks are featured. Blast Caster, Battle Axes and Funchunks are Omega Builds, only usable as special attacks when a bar is filled. Agent Six assigns missions throughout the game and when a mission is completed the player may go back and play the mission again, attempting to beat Six's high score. Additional collectibles throughout the game can be found as E.V.O. DNA samples for the main female protagonist, Doctor Holiday. A monkey E.V.O., Bobo Haha serves as a partner for the title character, Rex in the "Rex Ride" level. Upgrades can be bought for Rex's weapons, health and Omega Builds, using "Nanites".

Plot
Rex must prevent Van Kleiss and his minions from gaining unprecedented powers that could destroy the Earth. Multi-Head, Biowulf and the Heart of Aquania appear in the game, as supporting villains.When Rex goes on a mission to defeat the EVO, Multi-Headed EVO, he discovers a piece of a blueprint which later is stolen by Breach.
