Murchadh appears to have been the first to bear the surname Ua/Ó Flaithbheartaigh/O'Flaherty

 but his tenure as chief was short. The annals state Murchadh Ua an Chapail, i.e. Ua Flaithbheartaigh, and Niall, son of Muirgheas, two royal heirs of West Connaught, were slain. No further details are given.
