Susan, Meredith's stepmother, comes over to bring Meredith groceries. Derek warns her that Meredith feels smothered easily, so she shouldn't overdo her attempts to care for Meredith. The Chief's attempts to flirt go badly; Mark offers to be his “wingman.”

Cristina informs Marlowe that she has set a date to get married to Burke, and he tries to ply her with surgeries. In order to put him off, Cristina tries to shove her relationship into Marlowe's face by acting overly sweet and loving with Burke, which surprises and unnerves Burke.

Meredith is uncomfortable with Susan's attempts to mother her, but Alex tells her she's just overreacting because she was “raised by wolves.”

Ava/Jane Doe's bandages come off, and the search for someone who will recognize her begins.Addison compliments Alex on his handling of the Jane Doe case, and he brushes it off. The news arrives that they've found a possible family.

Izzie decides she needs to confess to Callie in order to get rid of her guilt. George doesn't agree, and tells her that he needs her to back off, comparing their liaison to when he had "tragic" sex with Meredith.
Izzie is shocked when the adoptive parents of her daughter, Hannah, show up at the hospital. Hannah has leukemia and needs a bone marrow transplant, and they are hoping Izzie will be a match. But Izzie has a request of her own: she wants to meet Hannah.

Derek gets angry with the Chief, who had promised Derek the position as his successor when Derek moved from New York. He asks if the Chief will support him with the Board, since his interview didn't go very well.

Izzie is overwhelmed with the prospect of meeting her daughter when she's unprepared. Bailey tries to support her. However, Izzie is devastated when Hannah doesn't want to meet her, at least not now.

George is the only one of the interns to notice that something's going on with Izzie. Izzie begins the bone marrow transplant, with Bailey there to hold her hand. Though they're keeping it a secret, George finds out and goes to support Izzie, but he wants to know who the transplant is for. Callie arrives looking for George and is upset when she finds out that he's ditched her to be with Izzie.

Jane Doe/Ava is tentatively identified as a woman named Shannon, and her parents show up at the hospital. She wants to go home with them, even though she doesn't remember anything. Alex tells her she'll need to wait, but she pressures him to get her released. Shannon's mother tells Alex that they are mistaken, and Ava is not Shannon. Alex is reluctant to let her go.

Susan shows up at the hospital, and Meredith gets upset. She tells her to stop trying to mother her. She doesn't want to be Susan's daughter, or her charity case.
The Chief tells Derek that the real reason he will not vote for him because he promised Ellis that he would look after Meredith, and Derek won't be able to have a real life if he becomes the new Chief. Derek gets upset, and goes to talk to Bailey—he is beginning to see that Meredith is a liability, just as Bailey originally warned him. Bailey tells him that success means nothing if you don't have the people you love.

Cristina continues to pick at Marlowe and shove her wedding in his face. Marlowe, Preston, and Cristina begin the surgery, and Marlowe insults Preston and tells him that he can only observe and never should have scrubbed in. After the surgery, Cristina tries to talk to Preston about it, but he won't engage. Marlowe overhears their conversation and criticizes Cristina for building up Preston's ego. Cristina tells him that she's learned to care about other people. Marlowe admits that he came to Seattle Grace because of her, but then says that it was a mistake because the job is beneath him and Cristina is not the woman he thought she was. At home, Preston tells Cristina he's glad that Marlowe is leaving, because he wants her back to her old self.

After the transplant, Izzie tells George the truth about Hannah and why she's having the transplant, and how hurt she is that Hannah won't meet her. She also tells George that—whatever else it was—their having sex was nothing like when he had sex with Meredith, and that they can't be friends. He says he understands. George and Izzie go to watch the transplant through the observation window, but Izzie can't bring herself to look. George describes what Hannah looks like to her, and convinces Izzie to go and look for herself.

Callie finds George, and wants to know why exactly George ditched her for Izzie. George lies to her, not wanting to tell Izzie's secrets, and Callie leaves upset.

Alex goes to tell Ava that Shannon's parents made a mistake, and Ava gets upset. She lashes out at Alex, crying that he only wants to keep her there for selfish reasons, and that her name isn't really Ava.

That night, Meredith is at home, futilely trying to call Derek (who is sitting in his trailer ignoring her calls) when Susan shows up. Meredith is rude, but Susan insists on coming in. She reveals to Meredith that she blames herself for Thatcher not being involved in Meredith's life, and says she should have pushed him harder to maintain a relationship with Meredith. Her overblown attempts at mothering Meredith are as a result of her guilt over this. Meredith tells her it's okay, but it's just new to her.

The Chief figures out that Mark's attempts to show him how to flirt and meet women is because Mark thinks that if the Chief succeeds due to Mark's efforts, the Chief will vote for Mark. The Chief hits on a woman who turns out to be a college student and feels embarrassed. Addison shows up and tells the Chief to practice flirting with her, and they dance in the middle of Joe's bar.
