From July 19, 2011, Lion Air has had to ground 13 planes due to sanction caused by bad on-time performance (OTP) until Lion Air can fulfill at least 80 percent of OTP. The transportation ministry recorded that Lion Air's OTP of 66.45 percent was the worst out of six airlines in an assessment conducted from January to April 2011 at 24 airports nationwide.

No injuries occurred, the airplane sustained minor damage.
  • On 15 february 2011A Lionair Boeing 737-900, registration PK-LHH performing flight JT-295 from Medan to Pekanbaru (Indonesia) with 218 people on board, overran Pekanbaru's runway 36 and came to a stop with all gear about 25 meters off the paved surface while landing in Pekanbaru around 17:00L (10:00Z) in drizzle. The passengers disembarked onto soft ground via mobile stairs.
  • On 23 october 2011 A Lion Air Boeing 737-900, registration PK-LHO performing flight JT-673 from Tarakan to Balikpapan (Indonesia) with 198 passengers and 7 crew, overran the end of the runway 25 with the main gear about 20 meters off paved surface (60 meters paved runway end safety area) when the aircraft landed in Balikpapan in heavy rain and strong gusting winds. No injuries occurred, the passengers disembarked normally onto soft ground.
