Episode Summary

Day 20: At the start of the episode, it is shown that the woman (Shannon Lucio

) whom George O'Malley saved is sat outside the hospital, and hasn't moved. Lexie is still sat by the bed of Clara (Zoe Boyle), who tried to kill herself, after she informed doctors that they should have left her to die. Cristina and Owen also look to be still abstaining from sex.

Day 21: Clara receives a postcard from the friends she was taking the trip with, saying that they are in New York. Clara expresses her jealousy, but says that she thinks Lexie should go home, as she no longer feels suicidal. She also refers to Lexie's persistent avoidance of moving in with Mark, saying that if she goes through with it, Clara will let Lexie call her mother.
Clara is then informed by Doctor Bailey that she has an infection, and needs surgery. Clara rejects the surgery, as she doesn't want to take the risk of something going wrong. This is made worse when Cristina explains that the worst case scenario is that she can never use the bathroom on her own again.

Back in the ER is the mother (Martha Plimpton) and her son Andy from the previous episode, who is experiencing chronic pain with no explanation as to why. Doctor Robbins and Alex ask for a 3D MRI, but the chief rejects it on a count of it being too expensive.

Doctor Bailey finds Cristina, and argues with her about how much she should have disclosed to Clara about the risks of the surgery, saying that she is "killing her patients". She then bans Cristina from speaking to any of the patients.

Cristina and Meredith go to visit Izzie in the cancer ward, where she tells them that she's feeling better. An unusually tactless Cristina makes insensitive remarks throughout the conversation, which Meredith attributes to her not dealing with her grief properly, and the fact that Owen is withholding sex. Izzie admits that Alex is doing the same thing, saying she thinks he's afraid.

Perhaps due to Clara's bet, Lexie is moving in with Mark, but doesn't appear to want to leave her things in the apartment. Their conversation is interrupted by Callie, who asks Mark's opinion on her outfit, and proceeds to undress in the hall in front of Mark, much to the chagrin of Lexie.

On the way to his board meeting, the Chief is talking to himself, clearly worried about the possibility he may get fired from his position. Distracted, he runs a red light, and is hit by a car. He is taken to Mercy West, where he is looked at by Callie, who is in no rush to have him discharged, as she is still annoyed that he fired her. She allows an intern to do his stitches, so it'll take longer. During conversation, the Chief discovers Mercy West is also having money problems.

Day 23: Briefly, it's shown that Amanda is still sat outside the hospital. It then cuts to Owen and Cristina's therapy session, where Cristina is listing off all the things she knows about Owen in order to prove she knows him well enough for them to have sex again. Dr. Wyatt Amy Madigan

says that she doesn't need Cristina to know pointless facts, she just needs Owen to be comfortable talking about his problems, otherwise he won't deal with his PTSD.

Lexie, whilst writing an email to Clara's mother, confronts her about refusing surgery, typing an email to her mother telling her Clara is going to die. Although Clara repeatedly calls Lexie a bitch, it appears she has finally consented to the surgery.

Back at Mercy West, Doctor Robbins has referred Andy and his mother to Callie, with hopes that they'll do the 3D MRI there. Andy's mother admits that she never sleeps anymore, and feels like a terrible mother, and begs for the test until Callie agrees.

In Clara's surgery, it appears that the disease has had time to progress because the surgery was put off. Doctor Bailey berates Cristina for telling Clara about the risks, rejecting her offer to help, saying she'd "done enough". After the surgery, Cristina defends herself, saying she was only doing as the patient asked. Doctor Bailey kicks her off of her service, saying she is disrespectful.

Mark and Derek talk about why there are so many people in suits coming into the hospital. Mark says that people are starting to get worried, and asks Derek if he knows what's going on. Derek jokes and says that the Chief is retiring and that he will be becoming Chief; actually, he has no idea what's going on.

At home, a very stressed out Arizona awaits the return of Callie, knowing she will be in trouble for referring the patient to her in order to get what she wants. Despite having run the test, however, it didn't show anything noticeable; they still have no idea what's wrong with him.

Returning to Meredith's house, Lexie runs in on Derek and Meredith having sex in the kitchen. Conversing with a naked Derek, Lexie inquires as to whether their post-it wedding is for real, which Derek says it is.

Day 30: Outside of what was Derek's trailer, Alex - still apparently avoiding Izzie - tries to diagnose a patient whilst she asks him to come inside and take a break, because she misses him. Alex rejects her, so Izzie explains that she's praying for a brain tumour so she can hallucinate George back, like she did Denny. Alex cruelly bites out that mentioning George is hardly seductive, and Izzie leaves.

Day 36: In the morning, Callie arrives at Lexie and Mark's apartment, and nonchalantly walks in on Mark in the shower to ask about the Chief. Neither of them seem too perturbed about this, but Lexie finally snaps and inquires after a definition of Callie's sexual orientation, telling her that she's so hot that she's worried that her 'hot naked boyfriend' will start liking her again. Callie tells Lexie that he no longer looks at her that way, since he met Lexie.

In the Chief's office, Derek has been waiting to ask what is going on with the hospital. The Chief accuses him of going behind his back to Jennings, an idea with Derek rejects, saying he's the one that has 'got his back'. After leaving the office, Derek runs into Doctor Robbins, who says she thinks Andy has a tethered spinal chord, and she needs to run another test. Although sceptical about the test due to the rareness off the affliction, once Derek hears that the Chief rejected the request for the test, he offers to order it himself.

In Clara's room, Clara is having physiotherapy to learn how to stand again. She's only just up when she asks to be put down again, and the physiotherapist says that what she's going through is now the hard part. This seems to distress Clara slightly, showing that Clara is clearly not happy.

In Andy's MRI, Doctor Robbins admits that she now understands why everyone calls Derek 'McDreamy'. She also congratulates him on getting married, which provokes Alex to snap that Derek isn't really married, because there was no actual wedding, just some 'hoaky crap on a post-it'. Derek retorts that at least he has consummated his marriage, admitting that Meredith had told him about his avoidance of Izzie, and Alex should consider that the next time he 'judge's [his] post-it'.

In the resident's lounge, Meredith attempts to have a serious conversation with Cristina, admitting that she has been using work and sex as a distraction, and hasn't cried about George yet. She also attempts to bring up the topic of the girl sat outside the hospital, but Cristina changes the topic over to the lack of sex between her and Owen. Meredith leaves, and Lexie appears, saying she's worried about Clara. Cristina calls her Ceviche again, and Lexie storms out offended.

With the results from the MRI, Derek reveals that Andy does indeed have a tethered spinal chord, and that with a micro surgery, he'll be fine. His mother breaks down.

In an appointment with a cancer specialist, Izzie and Alex have been appointed a new doctor, as her cancer is no longer severe enough for Doctor Swinder. The new doctor reveals that the cancer is still there, but it is getting better, so she will continue with her medication and hope for the best. Alex asks for a timeline, which the doctor cannot give him; he says that Izzie is lucky to have survived, and that she will have to continue living with cancer.

Day 37: In Andy's surgery, Derek offers the opportunity to cut the spinal chord to Arizona, as it's her success that means he can live a normal life.

Hunt goes to speak to Clara, saying that she needs to do physical therapy, or she'll never be able to work the muscles. She ignores him, clearly depressed, until Lexie snaps, and tells her that everyone is calling her Ceviche, a type of chopped fish, and that she can't be known as that. This makes Clara laugh, snapping her out of her depression.

Day 39: Izzie is outside the hospital, telling Meredith that she didn't expect to live, and she doesn't know how to live with the cancer. She then notices Amanda still sitting outside the hospital, and goes over to her, telling her to get up. She shouts at her to go get a life, saying that George gave up his purpose and his chance to do things to save her, and she's being ungrateful by not living her life.

In their therapy session, Doctor Wyatt is listening in on Owen and Cristina talking about work. Cristina is called out on calling Clara Ceviche, which makes the two of them laugh when she compares it to Owen choking her in her sleep. Owen admits that he didn't want to choke her, that he was dreaming about fighting for his own life, and didn't mean to hurt her. Once Cristina tells him that she believes him, Doctor Wyatt commends them on 'making a start.'

Back in physical therapy, Clara is starting to walk on her own. Once she makes it, Clara asks Lexie to hell her mother, which she agrees to do. This provokes Lexie into taking her things to Mark's apartment, effectively moving in with for real.

Alex comes back to the trailer, and finds Izzie, who has set up candles around the room, and demands that he take off his pants. When he rejects her, she argues with him, saying he needs to be her husband. Alex snaps, saying that she died in his arms, and left him instructions not to save her, and now he's scared to do anything because he won't survive if he loses her. He blames Izzie for this, saying "You made me love you, you made me let you in, and then you freaking die in my arms." Izzie kisses him.

Miranda and Derek are riding in an elevator together, when Derek stops the elevator and forces Miranda to tell him whats wrong. She says that everything is too hard, and that she can't care anymore, because by treating her interns like her children, she keeps feeling things for them when bad things happen. Derek restarts the elevator.

Cristina and Owen, now at home, appear to be trying again at having sex, before Owen stops them to tell Cristina that he's scared because his problems are now her problems, and he doesn't want to hurt her again. Cristina suggests he spend the night in the bathtub, so he can't do anything in his sleep.

A janitor comes to empty George's locker, surprising Meredith. A while later, when Derek comes to find her, she's sat, finally crying about George's death. In Joe's bar, Callie arrives to meet Arizona, talking about how an idiotic intern at Mercy West reminds her of George when he was an intern, and after Cristina and Owen finally have sex, Cristina is hit with the realisation that George O'Malley did indeed die.

Day 40: The voice over of Meredith says "There are 5 stages of grief." Each stage is used to point to someone throughout the Chief's speech. Alex is denial, Derek's is anger, Miranda's in bargaining, Lexie's is depression, and the Chief's is acceptance. During the speech, everyone finds out that Seattle Grace will be merging with Mercy West hospital, and that a lot of people will be losing their jobs.
