Summary

The episode starts with Alex pacing outside Izzie's room. Callie and Arizona are dating. Owen falls asleep at Cristina's after staring at the blades of her ceiling fan. This prompts a PTSD event in which he chokes Cristina. Meredith comes over and Owen is a mess. Cristina tells Meredith that it was an accident. Owen cries apologetically.

Derek comes to Meredith's house with the engagement ring, but Meredith tells him he needs to focus on Izzie. Meredith is also frustrated that Cristina forgave Owen, and Alex is concerned about Derek's ability to operate on Izzie after his breakdown. George is upset that he was the last to know about Izzie's condition and asks to be going on Callie's service.

Bailey tells the other three that they are going in as her friends, not as her doctors. Izzie's eggs will be harvested as a way to allow her to have children if she wants them, but they have a better survival rate if fertilized. The Chief tells Alex and asks for the cup. Cristina hears some really bad news about the chemotherapy treatment plan but does not want Izzie to know pre-op. She holds the oncologist off by telling her Izzie is in radiology.

Owen is spooked by helicopter blades and cannot retrieve the patient. Meredith tries to convince Cristina to walk away from Owen, but she defends him as being wounded.

Karev is working on an elderly woman named Joan who was given one month to live three years ago, but every time her family comes to say goodbye to her, she stabilizes and lives. They say it's time for her to go as it's getting expensive flying around.

Derek tries to talk to Owen about PTSD and therapy, but Owen tells him that he needs to spend less time worrying about his nightmares and more time focusing on his breakdown.

Alex vents to Meredith about Izzie's hallucinations and how he didn't even realize she was sick because of all the crazy women who have been in her life.

Miranda quizzes Izzie on procedures telling her that she'll need to be able to remember them a year from now to teach the interns. Derek prepares by himself for her surgery.

At lunch the four realize that nobody has been up to visit Izzie. Miranda spends time with her talking about her surgeries. Callie laments to Arizona that she'd wished Izzie would die when she was with George. Arizona reassures her that she's changed her mind and is praying for her to live now.

Karev brings Aunt Joan back one more time and she tells him that people are better than no people and that her family keeps coming back. Alex runs to find Izzie but she's already been taken to surgery. Izzie tells Bailey that the scarf she made was for her.

Callie comforts George who is still upset that he tried to connect with Izzie but that she ignored him and told Cristina. Callie tells him that he owes her for cheating on her, and she knows he cares if she lives or dies. Karev lets the interns stay and loiter during the surgery. Bailey assists Derek and defends him to Dr. Swender to keep him encouraged.

Aunt Joan's family becomes emotional when she finally passes.

Bailey tells the others that Izzie is fine, no thanks to them, and that they need to do better tomorrow.

Cristina and Owen have sex.

The Chief blocks Meredith from an elevator so that she's forced to wait for one holding Derek. It is filled with memories of their surgeries together. He tells her that she got him into the O.R. and that she survives for everyone. Derek proposes to Meredith for the third time in the infamous elevator, and she accepts.

Cristina breaks up with Owen because she's afraid to fall asleep in his arms. Owen agrees to get scans for his PTSD.
