Episode Summary

This episode is a Grey's Anatomy and Private Practice crossover, Addison and Naomi, from Private Practice, take Archer, Addison's brother, to Seattle Grace with the goal that Derek can somehow treat the parasites which are attacking Archer's brain. Addison and Naomi arrive at the hospital (later followed by Sam) with Archer, and Addison asks Derek to put aside their former negative feelings and become a miracle worker on her brother. He doesn't think he can pull it off but for his ex-wife, he'll do his best. Derek rises to the occasion and ultimately removes the parasites from Archer's brain, though there is a moment of question. Unfortunately Derek does not have the same brilliance during Jen's aneurysm surgery, when he nicks part of her brain and things go significantly downhill from there.

Many old medical school pals cross paths in this episode. Sam reconnects with Derek and admits he got a mid-life crisis ear piercing. Derek assures Sam that Meredith is not a midlife crisis girlfriend.

Alex begins to suspect something is really wrong with Izzie. Izzie's game for the interns is useful because it gives Alex another indication that something might be really wrong with her. In addition to the dead-boyfriend thing, she now doesn't seem to be seeing very well. Alex still has feelings for her, and seeks Addison's advice.

Sadie quits the intern program, in part due to George exposing her lack of knowledge during the game. Sadie looks to Meredith for help, but Meredith tells her to basically grow up instead.

Sam suffers an asthma attack, and his friends believe it's because he's freaking out over Naomi being with another man. In actuality it's from Bailey giving him an inhaler he was allergic to.

Cristina finds out about Owen's mystery woman, who turns out to be his ex-fiancé, whom he dumped in a very short email. His ex-future father-in-law is dying of cancer and has yet to tell his daughter about his condition. Owen has yet to tell this same woman that he's back from the war. In fact, Owen hasn't told anyone he's back, not even his mother, because he is afraid of them seeing the hardened person he's become. Owen ends up having to tell his former fiancé about her father's condition.
