Why was General Archer referred to as "Tom Archer" in Mary Chestnut's diary?

What exactly did General Archer die from? It is stated that his health deteriorated by imprisonment and harsh enviornment. After reading his letters to his family up to a few days before his death, it was clear he did not think his condtion too serious.
