LT Steven M. Graves solicited data from the Philadelphia USCG office, specifically their positions of the 144 containers of arsenic tri-oxide (Operation Toxic Look 1992), and used it to correct the plot generated by sailors of Tactics Division, so that position error was less than first reported. This falsified information made it appear that the squadrons capabilities to locate and report an object was better than it actually was. For this lie, LT Graves received a USCG Commendation Medal and was promoted to LCDR.
