I am applying for 'Adjustment Status' of my visa here in the U.S. and I am required to submit a medical records that would include my immunizations. I was born 1962 and during that time most mothers do not maintain a record of their child's immunization. My mother said I got my shots from the Health Center.
Does the DOH have a centralized records from all the local Health Centers where I can possibly secure mine?
